Before the incident
[REDACTED] was in the process of removing a pallet he had picked. [REDACTED] was going forward on the forklift to remove the pallet from the picker. [REDACTED] then put his forklift into reverse he looked back and made eye contact with [REDACTED]. [REDACTED] proceeded to go forward again to drop the pallet into staging. When [REDACTED] put the forklift in reverse he looked back losing site of [REDACTED]. [REDACTED] thought [REDACTED] was going to go in a different direction so [REDACTED] thought he had enough time so he proceeded to walk behind [REDACTED] while the forklift was in operation. That was the same time [REDACTED] lost sight of [REDACTED]. When [REDACTED] lost site of [REDACTED] he stopped to get off his lift and noticed [REDACTED] getting up off the floor.
