Employee was monitoring the tote stacking machine to ensure the machine was operating smoothly.

Employee noticed that some totes got stuck on the tote stacker and in violation of company safety policy stepped onto the rollers where the stacker doors open and tried to fix the totes without following proper lockout tagout protocol. Another employee did not see that the employee had gone in there and closed the doors of the stacker which caught the employee 's right arm between the totes and the doors. He hit his right elbow.
