Before the incident
TM was using a slide hammer to install coupling bushings on a 106 Wheel when the TMs finger was pinched. The TM was lifting up on the drive bar when the 5 lbs. handle weight which was not fully engaged onto the main body of the drive bar lifted up allowing the TMs left index finger to enter between the gap. As the TM lifted up on the drive bar the momentum carried the handle weight up exposing a partial gap on the end of the drive bar allowing the TMs left index finger to enter between the gap. As the upward motion stopped as the TM paused before beginning the downward motion on the drive bar the 5 lbs. handle weight dropped down and pinched the TMs left proximal phalanx segment resulting in a laceration.
