Before the incident
Operators were working to remove transfer pipes from the wall in the [REDACTED] room during a routine shutdown to clean transfer pipes. The transfer pipe was pulled from the wall mounts and was being handed down from the raised platform in the [REDACTED] room. The operator handing down the pipe and asked if the EE had control of the pipe. The EE responded in the affirmative and the pipe was then handed down to the lower level. When the operator was taking control of the pipe the pipe slipped through their hands and struck their left foot directly behind the safety toe which resulted in a fracture of the Great Toe of the left foot.
