EE was walking down a hallway under construction to answer a call light. EE stepped off of the carpeted flooring where it was safe to walk and stepped onto the unfinished floor. The unfinished flooring had a wet spot that EE slipped on and fell to the floor. EE fell onto both knees both hands and hit her head on the wall. [REDACTED] staff RN and [REDACTED] [REDACTED] Director witnessed the fall. [REDACTED] provided first aid assistance and looked at areas of injury. [REDACTED] and EE agreed that further medical attention was likely not needed at this time. EE declined my offer to provide medical care for her at Concentra Urgent Care. EE returned to work following a brief break.
