What happened
With another nurse turning a patient who was unable to assist on [REDACTED]. Halfway through the turn pt. screamed stop turn stopped but patient 's full weight was in mt hands straining on lower back. Rest of shift back was sore but not in pain. Has the [REDACTED] two days off pain improved. Worked [REDACTED] back was sore but tolerable new admission needed help transferring to bed. I held feet and that set off pain end of shift and return home was unbearable. [REDACTED] pain returned to tolerable 4 [REDACTED] and was reported to AM supervisor went to ED for evaluation and then sent home.
