Unclogging a wheel from the conveyor trailer
What happened
TM was working as the quality liaison at SHAP. TM noticed a wheel get stuck on the automatic conveyor and moved to unclog it. TM stood on the trailer with a stick to free the stuck assembly. The trailer then abruptly pulled away causing the TM to fall between the platform and trailer. TM reported to the quality manager but did not want medical treatment. SHAP 's EHS team then required the TM to receive medical attention as their condition over the span of a half hour significantly worsened. EMS was called by SHAP safety team and arrived shortly after [REDACTED].
