Two maintenance workers were troubleshooting a faulty actuator. They removed the end covers to check if there was air leakage around the piston head. They connected air hose to the side of the actuator and applied pressure. At that time the piston head blew out of housing and struck the maintenance worker in the chest causing Blunt force injury to right abdominal area liver laceration
