What happened

Employee took their right index & middle finger and placed their fingers on the end of a piece of material that was .390 inches in diameter and 69.5 inches in length. Employee then with their two fingers pushed the piece of material towards the roller table of the horizontal bandsaw. While the employee was pushing the material towards the saw’s roller table the employee’s right index & middle fingers were positioned within the bandsaw’s vice grip mechanism. With their fingers within the vice grip mechanism the employee placed their left hand on the vice grip control switch and turned the switch to the “left”. By turning the control switch to the left the employee engaged the vice grip mechanism causing the vice grip to close on the employee’s fingers up to the second knuckle.
