Was going to use the toilet- backed up to the toilet- slipped and fell back into the toilet
What happened
[REDACTED] asked to use the restroom- was one of the checkout clerks. She came back to the front [REDACTED]. She stated that she fell in the bathroom. When in the restroom- she was backing to sit on the toilet- slipped in a puddle that was directly in front of the toilet- and fell onto the toilet. I then checked to see if there was any moisture in the stall she fell- and the floor was dry. Employee was dry and I took pictures. She stated that she cleaned up the puddle with bath tissue and flushed it. She says that she has pain in her lower back and in her hips.
