Repositioning precast concrete girder side form with the assistance of an overhead crane
What happened
While moving an approx. 30 ' steel side form of the girder form with an overhead crane the side form became stuck on the base of the girder bed. The employee was standing on the inside of the girder bed when he attempted to shake the side form free which caused the crane to release the side form and roll inward onto employees feet.
