On shift date 12 09 25 at around 02:00AM (12 10 25) there was an incident involving our forklift operator Oscar Jimenez employee number 1823773 injuring his left foot. He came into the office and signed a waiver. On shift date 12 10 25 Oscar Jimenez told me that his foot pain has increased and would like to seek medical treatment. Associate stated he felt last night it was a minor thing. He thought he could self-care. However when he woke up the pain was intense and can barely walk. I left [REDACTED] know via phone (voicemail) and I told Gina about this incident. I had Oscar Jimenez call the Nurse hotline at the start of the shift. The Nurse recommended Oscar to go to urgent care and Oscar agreed. The Nurse gave him claim reference number T- 00971541-1 for this incident. Oscar went to the clinic at 07:55pm and came back at 10:30PM with restrictions. Oscar was sent home at 11:00PM and was told that [REDACTED] or Gina would reach out to him tomorrow regarding his RTW status.
