What happened

[REDACTED] 702411 was working on the south Vemag [REDACTED] raw completing a weekly PM. At or around [REDACTED] [REDACTED] called he came walking into formulation from [REDACTED] raw holding his right hand wrapped in cotton gloves. I asked what happened and [REDACTED] proceeded to say I messed up I messed up bad. My finger is gone. I messed up and was not locked out. At that point I made a call on channel 1 to [REDACTED] as to her location and I was bringing a member to her for medical assistance. I helped hold [REDACTED] hand as he held direct pressure to his injury. As we walked through formulation I grabbed extra gloves from my cart to contain any extra spillage. I asked [REDACTED] on our walk to RTE team leader office what happened and how he felt. [REDACTED] said he felt ok for the moment and explained he was working on the PM the Vemag was faulted and as he cleared the fault it started and his finger was cut off by the drive belt and pulley.
