Both employees were working on maintenance on a forklift and they removed the tires to lubricate and clean it so they used a jack to lift the forklift to remove the tires. The injured employee was going to put the tire back after finishing. While doing so the jack lost power and the forklift went down. Leaving their hand between the forklift and the tire.
