Before the incident
The employee was performing a task to disengage the quick disconnect on the gas-head line for the [REDACTED] tundish on the far left side.
What happened
They were pushing the disconnect collar in a downward motion with their right hand whiling pulling the gas hose with their left. This activity is performed while standing on a diamond plate platform. The difficulty to release the disconnect lead the operator tugging on the hose. The tugging motion caused the operator 's footing to slip backwards. When falling backwards the operator 's right hand was wedged between the metal pipe and the tundish coil.

The operator sustained a closed non displaced fracture of the shaft of the second metacarpal bone of the right hand.
