Clearing a jam from the clip press

Employee was making clips on the clip press and the press jammed. Employee did not turn off press or take foot off actuator pedal. Additionally the guard was not properly adjusted. Employee reached into the machine to clear the jam. His hand slipped and he pressed the actuator at the same time his hand slipped causing the press to cycle catching part of his left index finger causing a laceration. Employee noted he had cleared jams earlier in the day and had followed proper procedure to turn off the machine etc but did not do so this time.
