[REDACTED] was in process of unloading a live packaging trailer.

As [REDACTED] was in process of unloading a live packaging trailer the hook for the trailer locking system failed. The driver had not set his brakes on the truck or trailer allowing the trailer to glide away from the door. The dock lock system failed to hold the trailer. [REDACTED] was coming out of the trailer when he realized it was moving and attempted to drive back into the trailer. The rear tires of the forklift were on the dock plate which gave way when the trailer moved away from the door causing [REDACTED] and the Forklift to fall to the ground outside of the building.
