Before the incident
Cutting metal using a plasma cutter he was done and putting the torch away by wrapping the cord onto the cart that held the cutter and cord.
What happened
While holding the Plasma torch to wrap the cord to put it away. Wrapping the cord caused the torch to turn in his gloved hand and he grabbed the torch to stop it from turning which caused it to activate while it was pointing towards his palm.
