Swaging a flemish eye

We received a 1500 ' reel of 7 16 6X19 with 5 ' of the inside end of the reel exposed so we could install a flemish thimble eye. Because the tail was this short we had to lift the reel on the forklift to get the eye into our 500-Ton press. The reel had very little flange exposed past the coils of wire rope so the forks had only a small purchase on each side of the flange. The rigger closed the dies on the sleeve to stabilize it while he placed his left hand on the rope to the left side of the dies and pulled the rope to tighten the eye on the thimble. While adjusting the eye the rigger asked the forklift operator to shift the forks right to give him some more room. This shifting of the forks removed the slack in the rope and cause the reel flange to slip off the fork. When the reel slipped the riggers hand was still on the rope and the sleeve was held by the dies. The riggers fingers were pinched between the wire rope and the surface of the press.
