EE alleges about [REDACTED] while doing room checks he opened the door to check on the resident and the resident pulled a cord which slammed the door shut on his hand injuring his L hand index middle ring and little fingers.

EE calling back from exposure that occurred [REDACTED] about [REDACTED] while he was working at the homeless shelter doing room checks walking around the building cleaning. When he came back into work on [REDACTED] ([REDACTED]) he noted some spots on his legs and arms that itched. On [REDACTED] he did a Telehealth visit with HealthPartners who told him he has scabies. EE was given prescription medication and after making a couple phone calls to TeleHealth he was given a note to return to work. EE states he did not get the second dose of the medication he was told he needed to take. EE denies any new or worsening symptoms. EE needs to be seen for the second dose of medication and for a current note to return to work. Provider given. EE was told by his HR that he needs the WC claim number in order to return to work. WC Insurance information given to EE. No manager available to close call with EE agrees to inform supervisor of Medcor Reference number.
