Before the incident
Employee was blowing off parts on Line 5 right after the wash rack. He was shaking the racks to get any access water off the parts before blowing. This is an improper technique which led to the rack coming loose.

Employee shook the rack jarring it lose causing it to fall and hit him on the left side of his face.
