Feeing wire at cardboard baler

On [REDACTED] at approximately [REDACTED] a forklift that was engaged in routine operations collided with a stationary cardboard baler causing an employee who was working near the baler to strike his head. It appears that the forklift was backing up along the aisle adjacent to the cardboard baler. The baler door was in the open position as appropriate as the employee who was injured was preparing to make a cardboard bale. The injured employee was not visible to the forklift operator as the baler blocked his view. While backup up the forklift may have hit the open baler door which pushed the baler towards the injured employee causing his head to strike a nearby stationary object injury causing the injury.
