He was holding an aluminum bar while facing away from the structure that was being raised.

When the structure was being raised the strap slipped off of the forklift and it caused the structure to fall down.

The structure hit his head when it fell and when he fell to the floor his hand was caught between two aluminum poles. He had a laceration on both of his fingers and the back of his head.
