CLAIMANT ALLEGES SHE HAD BEEN TEACHING IN THE DANCE STUDIO AND WAS ATTEMPTING TO USE THE RAMP TO REACH THE RAISED FLOOR. AFTER REMOVING HER SHOES SHE HAD ATTEMPTED TO WALK UP THE RAMP WHEN SHE HAD STRUCK HER FOOT AGAINST THE WOODEN LIP RESULTING IN A FRACTURE TO THE FOURTH TOE. THE CLAIMANT SUSTAINED A FRACTURE TO THE RIGHT FOOT FOURTH TOE. SHE HAD LATER TAKEN HERSELF TO RECEIVE MEDICAL ATTENTION.
