EE was loosening a ball nose on the extruder in order to remove it.

The operator was starting the line on Line 107. After the operator began to coat the part the ball nose on the blue extruder began to leak acrylic onto the coating die. The operator made the decision to change the ball nose. Because the acrylic had leaked through onto the nut of the ball nose it was difficult to loosen the nut. The operator had so much tension on the ball nose wrench that once he was able to get the wrench to turn toward him the wrench came off the nut and the end of the wrench hit him in his left cheek and left nostril. This caused a laceration on the associates face where the curvature of the nostril meets his upper lip. Shift Supervisor First Responder took associate to Lewis Gale Hospital in [REDACTED] where the associate was diagnosed with a fractured nose and laceration. The laceration was treated with Derma-Bond and associate released to go home.
