Before the incident
Employee was cleaning out the mortar mixer which included scraping dried material off the inside of the hopper.
What happened
Employee tipped the hopper forward to dump out the scraped material inside. To do so the employee had to hold open the cage guard with his hand. His hand slipped causing the cage to strike the employee in the back of the hard hat which then caused the employee to bite his lip.
