Several employees did not follow the proper procedure sequence causing the 4 individuals not to communicate property during this process. The 3 individuals were to signal to the operator wheels had been installed and container is ready to be lowered onto the track. Communication failed while [REDACTED] was still adjusting the wheel causing the wheel to scrape his shin.
