Using a 4Ibs hammer to align flange with the web at the tack station

4 11 Employee was using a 4lb hammer to align the flange with the web. The employee swung the hammer and missed the flange his follow thru from the miss caused his right index finger to make contact with the edge of the web. Upon evaluation the right index finger had full range of motion and minimal swelling. The employee denied the need for any medical treatment and returned to work. 4 22 Employee reported tingling in right index finger to supervisor and requested to see a physician. HSE Manager scheduled appointment for employee with Physician 's Quality Care OCCMED Clinic on [REDACTED] at [REDACTED]. 4 24 Physician drilled employees right index fingernail and prescribed an antibiotic.
