He was stepping backwards pulling the pallet jack and his right foot got stuck with another pallet jack that was parked nearby. His right foot got pinned between the stationary pallet jack and the moving pallet jack. He did not have enough time to stop the pallet jack because the floors were wet causing it to slip.
