What happened
From the bank wall while holding the water hose with his left hand he stepped on the top of the concrete wall retainer held onto the ladder rail with his right hand put his right foot on step number 4 of the ladder his right foot slipped and his right side of chest armpit got caught in the metal hook that holds the chain.
