The employee was preparing to oil the H-6 concrete pour buggy. He had transported an oil spray can to the buggy and placed it on the side platform. He then began ascending the fixed ladder system on the buggy while maintaining three points of contact.

As the employee ascended the fixed ladder the oil spray can he had placed on the side platform became unstable and fell towards him. As the object shifted the employee then released his handhold to readjust the can resulting in losing three-points of contact. The employee fell approximately 2 ' 6 from the ladder platform to the ground. During the fall his right lower leg twisted upon contact with the ground resulting in a broken lower right leg.

The falling oil spray can caused an attempted readjustment and consequential loss of grip resulting in subsequent fall.
