During a tool change operation an engineering technician improperly handled a metal singlearm gate designed to block aisle access Instead of locking the gate securely to the wall the employee left it extended and overhanging into the aisleway Shortly afterward a [REDACTED] driver was reversing down the aisle with proper use of his seatbelt As he was turning to watch the aisle behind him his right arm rested on the [REDACTED] cage and he was facing right unaware of the metal gate protruding on his left The overhanging gate struck the driver in the torso upper right arm area and contacted the protective cage
