What happened
As the employee was cleaning he removed his LOTO device to start the auger to clean the bottom of the auger. The employee continued to clean the equipment with the equipment running. As the employee was cleaning the water hose he was using slipped out of his hand and into the equipment. The employee reached into the equipment to remove the water hose his hand got smashed between the auger and feed conveyor.
