Before the incident
Per ED Report: [REDACTED] is a [REDACTED] female who works at this hospital and a history of HTN CVA arthritis osteoporosis who presents with left buttock and posterior head pain after a fall occurring 2 hours prior to arrival. She recounts how at [REDACTED] she was sitting on top of a stool while working as the HUC on the OB unit at this Hospital. Suddenly one of the legs of the stool gave way and she fell backwards. She hit the back of her head on an ultrasound machine and her left hip on the floor. She was able to get up after the fall on her own but felt dizzy and nauseous. She currently reports throbbing 5 10 pain to the back of her head mild left posterior hip buttock pain slight neck pain and slight back pain. She took Tylenol and ibuprofen shortly after the fall but continues to have a persistent headache. She is able to ambulate and bear weight on her left leg with no issues. - provider documented Workers Compensation paperwork completed; CT completed
