He was trying to remove a single empty bin out from being stuck other bins and the safety guide rails by placing it on the chain. He was able to move the bin a little. The bin then was stuck so he decided to manualy move the bin while the system was on. He then triggered the photo eye that activates the bin pusher. It moved forward and caught his foot against another beam and it stuck
