employee started the shift at [REDACTED] on [REDACTED] employee started getting things ready in molding helping the molders demold and get started. employees then got on forklift to start moving material for his area he noticed a small roll of uni material (5lbs) on the floor he got off the forklift to pick it up and place back on rack. Thats when employee bent down to get material and said he felt a sharp pain in back. notified supervisor immediate.
