Before the incident
TM had just arrived on site for his shift. TM was reloaded his boxtruck to prep for delivery.
What happened
TM was putting a loaded cart into box truck on dock door 4. As he was pushing the cart from the dock onto the plate and into the truck the front wheels of the cart got stuck in a groove on the floor of the box truck. The cart stopped and as the TM continued to push the cart it started to lean to the left. The TM was holding onto the cart as it fell onto his left knee.
