The driver was removing stacks of dough off the liftgate with his hand truck.
What happened
The driver put a hand truck under the stack of dough. When he tilted the stack back the dolly rolled forward the drivers hand slipped. When his hand slipped the stack fell backwards with the stack of dough. The stack of dough fell striking the TM.
