Before the incident

I arrived for hospice visits for 2 patients in home. I knocked at the door and there was no response. I opened door slightly and announced arrival. The family 's large dog pushed his head through the door opening came onto porch and aggressively bit my right upper leg. Patient 's adult granddaughter was sitting within site of incident at a table inside the house. She was aware that I was about to arrive and failed to put the dog out. She then informed me that the dog had bitten another home health staff member [REDACTED] previously. This information was not communicated by that staff member nor by my employer verbally nor was it documented as an alert in the patient record.
