removing a case of soda

On [REDACTED] Manager [REDACTED] received a call regarding an incident in the concession warehouse. Upon arrival [REDACTED] observed employee [REDACTED] sitting in a chair. [REDACTED] explained that he had been attempting to remove a case of soda that was stuck in the warehouse racking system which prevented him from retrieving an empty pallet. He requested assistance from other employees to ensure additional pallets would not fall while he was performing this task. After removing the case of soda the pallet behind the empty one shifted forward into the vacant space. In an effort to avoid being struck by the moving pallet [REDACTED] descended the ladder stairs he was on and jumped off the remaining steps. This caused him to fall to the ground injuring his shoulder. The ladder then fell onto his ankle and heel.
