Before the incident
Employee was working during early hours at the facility assisting in an early pour of concrete with the batch house dispatcher and batcher. Employee was asked to operate and drive forklift which was being used to lift materials to the top of the batch house and return forklift to warehouse.
What happened
Employee began driving forklift with forks raised up. The forklift hit a ditch in the road which caused the forklift to swerve. Employee was ejected from forklift cab and landed on ground. Top of forklift fell on employee 's right ankle.
