Employee began to have symptoms of COVID-19 on [REDACTED] and initially took a negative home test. Employee was still having symptoms and went to the doctor on [REDACTED] and received another negative test. Employee was still not feeling well on [REDACTED] and took another home test that came back COVID-19 positive.

Employee stated that they treated a patient on [REDACTED] [REDACTED] and [REDACTED] who later tested positive for COVID-19. Employee wore a blue surgical mask while completing treatments with patient. Employee also stated that the week their symptoms began they treated known COVID-19 positive patients in isolation rooms. While treating known COVID-19 positive patients they wore an [REDACTED] mask gown gloves and a visor sheild.
