Employees were repairing the dock plate. Based on investigation it appears that the hydraulic hoses were hooked up backward. When they moved the plate up to remove the safety the plate went down instead and with the weight and hydraulic force cause the safety pin to shear and the ramp collapsed throwing the employee out of the bay onto the ground.
