What happened
When the machine is in cycle mode it does not require the operator to go back to the machine and manually cycle the operation. The machine will cycle itself. Because it is easier to just have the machine cycle the operator was not able to remove her finger from the plate that hit her finger. This maintenance cleaning does not require the machine to be locked out but set into manual mode. A note will be added to the lockout tagout procedure
