WHILE EE WAS MOVING BACKWARDS NOT NOTICING A PALLET BEHIND EE GOT CAUGHT BETWEEN PALLET AND ELECTRIC JACK WHICH CAUSED EE TO FALL TO THE GROUND HITTING BACK AND TOES OF RIGHT FOOT.
