He dismounted the forklift went to the bunk of lumber admitted that he admitted that he did not tilt his forks back to keep them from falling placed his right hand on the top of the bunk then a row from the forklift fell onto his right thumb. He got back on his forklift and put his pull on a loading rack and then he reported it to me [REDACTED] Safety Coordinator
