Statement: While working today as usual the pallet jack failed to stop when engaging the brake then released the handle for emergency but kept rolling. The pallet jack was working normal until all of a sudden I found myself pinned between the jack and the racks. That 's when I ended up stuck and trapped by the pallet jack. I could not move until people working around me helped and freed me.
