What happened
Associate was traveling east on the northside of the building associate made a right turn intending to go into aisle L causing the left fork to make contact with the concrete cylinder the impact caused the rear wheels to be raised off the ground causing the associate to lean forward and come off his seat as he was not wearing his seat belt and hit his head on the top cross bar of the roll cage given the associate a laceration on front half of the associate head.
