Before the incident
[REDACTED] was staging 4ft pallets for put away on his [REDACTED] forklift along the inbound dock when he approached a manual pallet jack blocking the travel lane.
What happened
[REDACTED] got off his machine to move the pallet jack out of the way and didn 't see a piece of wood on the floor next to it. [REDACTED] stepped on a nail sticking out of the piece of wood which punctured through his shoe and sock and about a 1 4 inch into the bottom of his left foot behind the 2nd toe.
