When production manager and safety talked to employee she said her arm was numb & tingly. When asked if it happened at work she responded she didn 't know. When asked to complete the accident report she replied how do I do that I don 't know how it happened. We asked if she wanted to go to Workwell or to her Dr and she told us she received a letter from her Dr stating she would have to find another Dr. I (safety) called Workwell & explained situation. The person I talked to offered to send her to First Care if needed. I received a message from Workwell stating employee told them her arm went numb when picking up a piece of vinyl. She took a PTO day 7 27 she asked if she had to still do light duty that her shoulder was much better when she came in 7 28. I replied yes she still had to do light duty unless she contacted Workwell. 3 pcs of vinyl was weighed; 11lbs
